These Terms & Conditions (“Terms”) govern the purchase, usage, and benefits of the Nizzo Health Card, offered by Nizzocare Health Services. By purchasing or using the Nizzo Health Card, the cardholder agrees to comply with and be bound by these Terms.

1. Introduction

The Nizzo Health Card is a healthcare discount membership program, designed to provide eligible cardholders with special discounts on healthcare services offered by Nizzocare Health Services and its associated service providers.

The card is not an insurance policy and does not replace any health insurance coverage.

2. Definitions

• Company / Nizzocare: Nizzocare Health Services and its authorized partners.

• Cardholder: Individual or family member registered under the Nizzo Health Card.

• Health Card: Nizzo Health Card membership.

• Services: Healthcare services provided by Nizzocare Health Services and its network.

• Discount: Price reduction offered under the card, subject to applicable limits and terms.

3. Nature of the Nizzo Health Card

• The Nizzo Health Card is a discount card, not a medical insurance plan.

• It does not guarantee free treatment or cashless hospitalization.

• Discounts are applicable only on eligible services.

• Benefits are subject to availability of services and providers.

4. Eligibility

• The card is available for individuals and families.

• No medical examination is required to purchase the card.

• The card is valid only for registered members whose details are provided at the time of purchase.

• Benefits cannot be transferred to non-registered individuals.

5. Validity & Activation

• The card validity is one (1) year from the date of activation unless otherwise specified.

• Activation is done after successful payment and verification.

• Expired cards cannot be used for discounts.

• Renewal is required to continue benefits after expiry.

6. Discounts & Benefits

• Cardholders may receive discounts up to 50%, depending on the service.

• Discounts are applicable on:

  • Doctor consultations

  • Lab tests

  • Radiology tests

  • Surgeries

  • Home healthcare services

  • Other healthcare services offered by Nizzocare

• Discount percentages may vary based on:

  • Type of service

  • Location

  • Partner hospital or diagnostic center

  • Case complexity

Note: Discounts are not applicable on government taxes, consumables, implants, medicines, or third-party charges unless specified.

7. Service Limitations

• Discounts are applicable only on services arranged or approved by Nizzocare Health Services.

• Emergency services may not be eligible for discounts in all cases.

• Some premium or specialized procedures may have limited or no discounts.

• Discounts cannot be clubbed with other offers, promotions, insurance claims, or corporate schemes.

8. Surgical Services

• Surgical discounts are subject to:

  • Doctor evaluation

  • Hospital policies

  • Medical complexity

• Final discount amount is decided after medical assessment.

• Nizzocare does not guarantee surgery outcomes or timelines.

• Hospital admission charges, ICU charges, implants, and consumables may not be fully discounted.

9. Home Healthcare Services

• Home care services are subject to availability in the selected city.

• Discounts apply only on service charges and not on medical equipment or consumables.

• Service quality depends on third-party caregivers or providers.

10. Booking & Appointment

• Appointments must be booked through Nizzocare’s official channels.

• Walk-in appointments without prior intimation may not be eligible for discounts.

• Card must be presented (physical or digital) before availing services.

11. Payments

• Payment for services must be made directly to Nizzocare or its authorized service providers.

• Discounts are applied at the time of billing.

• No post-billing discount claims will be entertained.

12. Refund & Cancellation Policy

• The Nizzo Health Card fee is non-refundable once activated.

• No refunds will be provided for unused benefits.

• In case of cancellation before activation, refund decisions are at the sole discretion of Nizzocare.

13. Misuse & Termination

Nizzocare reserves the right to:

• Suspend or terminate the card without refund in case of:

  • Misuse

  • Fraud

  • Sharing card with unauthorized persons

  • Providing false information

• Legal action may be taken if required.

14. Limitation of Liability

• Nizzocare is not responsible for medical outcomes or complications.

• The company does not control treatment decisions made by doctors or hospitals.

• Nizzocare shall not be liable for indirect, incidental, or consequential damages.

15. Modification of Terms

• Nizzocare reserves the right to modify:

  • Discounts

  • Services

  • Terms & Conditions

• Updated terms will be published on the official website.

• Continued use of the card implies acceptance of updated terms.

16. Privacy Policy

• Personal and medical information will be handled confidentially.

• Data may be shared with partner service providers strictly for service delivery.

• Nizzocare follows applicable data protection laws.

17. Governing Law & Jurisdiction

• These Terms are governed by the laws of India.

• Any disputes shall be subject to the jurisdiction of courts in Maharashtra, India.
